Preexisting versus New-Onset Atrial Fibrillation in Septic Shock: Incidence, Prognosis and Management

Abstract

Atrial Fibrillation (AF) is an early and common occurrence in patients with septic shock when admitted to ICU, with a widely variable reported incidence. New-onset AF tends to be more common than preexisting AF, and it tends to portend a worse prognosis, although the scarcity of data prevents to establish firm conclusions on the subject. Clinical management also differs, with new-onset AF requiring a generally more aggresive approach, being mandatory to propmptly initate rate-control and sometimes rythm-control strategies, including both pharmacological and electrical cardioversión options. Anticoagulation should be pursued, although the risk of bleeding, which is increased in septic shock patients, should be taken into account
